We'll start by gathering your insurance information and contacting your insurance provider on your behalf. Our team will inquire about the specifics of your coverage for massage therapy, including any limitations, requirements, or restrictions that may apply. If your insurance plan requires a referral from a healthcare provider, such as a primary care physician, we'll guide you through the process of obtaining the necessary prescription. Your healthcare provider's prescription should include specific details, such as the ICD-10 format, a description of the body area to be treated (be specific about left or right side and upper/lower back), the recommended treatment plan (including frequency and duration of sessions), the type of massage to be performed, the time limit within which you must receive the sessions, and the provider's signature and date prescribed. What to Bring to Your Appointment: - Therapy prescription, if applicable - Insurance card or case number - Relevant diagnostic reports (e.g., x-ray or MRI) - Wear comfortable clothes that allow access to the body area being treated Your first appointment will include an interview and an extensive examination to assess your condition fully. During the interview, we'll review your medical history and current symptoms, including any previous therapy treatments you've had and their outcomes. The examination may involve assessing your range of motion, flexibility, posture, and mobility. If necessary, special clinical tests may be performed to aid in the assessment process. Following the interview and examination, I will explain the assessment of your condition and discuss the treatment plan designed specifically for you. Cancellation Policy

CANCELLATION POLICY We understand that unanticipated events happen occasionally in everyone’s life. Business meetings, project deadlines, flight delays, car problems, snowstorms, and illness are just a few reasons why one might consider canceling an appointment. In our desire to be effective and fair to all of our clients and out of consideration for our therapists’ time, we have adopted the following policies: • 48 hour advance notice is required when cancelling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 48 hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment. • No-shows: Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show”. They will be charged $45 for their “missed” appointment and future service will be denied until payment is made. • Arriving late Appointment times have been arranged specifically for you. If you arrive late your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session.
